Disclaimer - LOTS of information on this one, it is designed to cover all the bases. I spent a lot of time figuring this one out - hopefully the fonts will work for you. this is almost the kind of thing that you can just hand to somebody in passing and they will get the point - i tried to keep it simple but also comprehensive. feel free to change this as you wish, but know that i crafted this pretty intentionally and based on some experience.
